TOURNAMENT OF CHAMPIONS ENTRY FORM


Please read the information below before filling out your entry form:

• The CENTURY is a nationally ranked junior tournament open to players 20 years old and younger
• Cost of entry is $75 (includes both rounds of golf and lunch each day)
• The tournament is flighted by age and gender and is limited to the first 300 participants
• Entry form and payment must be submitted between April 1 and July 10, 2008
